Add the option to output as plain PGM

Previously no options were supported and the PGM produced was raw
binary. Not consebit supports options for help and plain to display help
or generate the output as plain ASCII.
This may introduce additional unhandled inputs.

Improve the program to use the whole color range

Also change the type that holds pair count to typedef for easier
upgrade. Default type is now unsigned long long.

Implement the functionality

It is quite a small project so it has only taken a single commit.
